Member Forms and Guides

The Trustee has designed a number of easy-to-use forms to enable members to easily transfer other benefits into AESuper, to advise us of changes in their insurance requirements, investment options, personal details and when leaving AESuper, of their benefit payment requirements. The forms for these purposes and others shown below can be downloaded from this website.

Member Forms
 
 

Employer Sponsored Member (Application) Form
This form is for members wanting to join AESuper who have been enrolled by their employer.  (This form should NOT be used by Self Employed, Individual or Spouse members as these members should use the Personal (Individual) Member Application Form).

Personal (Individual) Member Application Form
This form is for members wanting to join AESuper who are either Self Employed, Individuals or Spouse members.

Super Choice Form
This form is used by eligible members to instruct their employers to send their contributions to AESuper.

The AESuper Compliance Letter can also be downloaded to satisfy the documentation requirements of Question 3 on this form.  

Change of Membership Details Form
This form can be used by members to notify a change of address, employment status, name, employer or other changes to personal details.

Direct Debit Form
Used by members who wish to make Member Voluntary Contributions directly from their bank account.


Voluntary Contribution Form
Used by members who wish to make a member voluntary contribution, especially to be eligible for a Government Co-Contribution.

Spouse Member Contribution Form
Used by spouse members who wish to make a member contribution. (Note that a Personal (Individual) Member Application Form will have needed to be completed).


Salary Sacrifice Form
Used by members who have employers that allow salary sacrifice (pre-tax) contributions to be made.

Benefit Withdrawal Form
This form should be used by members who wish to transfer all or part of their benefit out of AESuper, have part of their benefit paid in cash (if eligible) or wish to retain their benefits in the Fund following resignation from their employer.


Insurance Application Form
Members who wish to increase their death and total and permanent disablement benefits should complete this insurance application

Investment Choice form
This form allows members to change their investment options at any time. You can change investment options for your current balance, future contributions or both.

Transfer In Form (Rollovers)
Members who wish to transfer benefits held in other funds to AESuper should complete this form.

Tax File Number (TFN) Notification Form
Members who did not notify AESuper of their tax file number when first joining who subsequently wish to do so should complete this form.

SuperMatch
Members who would like us to assist them in searching for any old super accounts can complete this form. If you have had a number of jobs and moved house a few times, you may be surprised at how much missing super you have.

Nomination of Beneficiaries Form
Used by members for nomination of dependents in the case of death
